This parasite enthralls and transforms people into mind flayers though the transformation does not occur immediately. The ship comes under attack from githyanki warriors riding red dragons shortly after waking. Mind flayer crew members teleport the damaged vessel to the Nine Hells where they face an army of Devils. The protagonist escapes during the fighting and steers the wrecked ship toward Faerûn until it crashes onto land. Other survivors emerge from the wreckage all implanted with similar tadpoles. The party seeks to remove these parasites through numerous avenues but every attempt fails to provide a cure. They are saved from the tadpoles' overwhelming psychic force by a Dream Visitor appearing inside Shadowheart's mysterious prism device. This prism is sought after by githyanki forces who want to control its power. The group travels to the Emerald Grove where druids and tiefling refugees fight off a goblin horde worshipping the Absolute cult. After deciding whether to sack or save the grove, players traverse mountains or the Underdark to reach Moonrise Towers in act two. There they confront General Ketheric Thorm using a Nightsong relic as an engine for immortality. The party scours the Gauntlet of Shar temple to find the Nightsong which turns out to be Dame Aylin daughter of Selûne. Orin the Red and Enver Gortash join Ketheric in controlling the Elder Brain known as the Absolute. These three villains manipulate everyone infected with a tadpole to rule the Sword Coast region.
Larian partially released an unfinished version of Baldur's Gate 3 into early access in October 2020 after revealing it at a Google Stadia conference in July 2019.
