On the night of Ai Hoshino's delivery, obstetrician Gorou Amamiya stands in a hospital room filled with the sterile scent of antiseptic and the hum of medical equipment. He is murdered by an obsessive fan who breaks into the facility, leaving his body cold on the floor before he wakes up as Aquamarine "Aqua" Hoshino, one of the newborn twins. Four years later, Aqua watches from the shadows as that same fan returns to murder Ai Hoshino right before his eyes. The child's memory of his past life remains intact, creating a ghost within the infant's body. His twin sister Ruby Hoshino carries the soul of Sarina Tendōji, another patient who had admired Ai. They grow up together in a world where fame hides rotting secrets beneath the glittering surface.
Author Aka Akasaka began writing Oshi no Ko after hearing complaints about the entertainment industry from streamers while working on the live-action film adaptation of Kaguya-sama: Love Is War. He decided to use his previous idea of being reincarnated as an idol's child but shifted the focus to the dark realities of modern stardom. Akasaka contacted artist Mengo Yokoyari immediately because she had previously dealt with the entertainment industry in her one-shot Kawaii. The two were long acquainted but had never collaborated on a full series until this project started.
