Grey's Anatomy

  • Grey's Anatomy began with nine star-billed actors, and by its 19th season only three were left: Ellen Pompeo, Chandra Wilson and James Pickens Jr. The medical drama follows surgical interns, residents and attendings at a fictional Seattle hospital. ABC has renewed it every year for two decades, which makes it the longest-running scripted primetime show currently airing on the network.

That run of two decades raises questions. How does a hospital drama hold an audience while its faces keep changing? Why does its title look like a misprint of a famous textbook? And what happens inside a typical hour, week after week? The first answer lies in how the show was pitched.

  • Shonda Rhimes has described herself as obsessed with the surgery channels. She and her sisters would phone each other to trade stories about operations they had watched on the Discovery Channel. What fascinated her was the odd intimacy of the setting, where doctors chat about their boyfriends or their day while cutting a patient open. When ABC asked her to write another pilot, the operating room felt like the natural place to set one.

Her stated aim was a show about "smart women competing against one another." She wanted viewers to feel they were watching very real women. In her view, most women on television were ideas rather than people, and they rarely got to be nasty, competitive, hungry or angry. They were usually the loving wife or the nice friend.

Francie Calfo, an executive vice president of development at ABC Entertainment, saw a gap in the market. The network wanted a medical show that stood apart from the others then on the air. Calfo said that while rival series sped up, Rhimes found a way to slow down, so that audiences came to know the characters. She also noted the show's strong female appeal.

The title is a pun. Henry Gray's classic English medical textbook, Gray's Anatomy, first appeared in 1858 and is still in print. The show swaps in the surname of its title character, Dr. Meredith Grey, played by Ellen Pompeo. Before broadcast, the team discussed calling the series Complications, but that change never happened.

Nearly every episode title is borrowed from a song, including "A Hard Day's Night" and "How to Save a Life." "It's the End of the World as We Know It (And I Feel Fine)" lent its name to two episodes. One title breaks the pattern: "1-800-799-7233," the number of the National Domestic Violence Hotline. Showrunner Krista Vernoff said that actor Giacomo Gianniotti, who plays Andrew DeLuca, suggested the change from the original name, "Four Seasons in One Day." With the concept settled, the writers still had to find the people to play it, starting with the role named in the title.

  • Rhimes has said she would have been hesitant to continue the series if ABC had not let her write characters without fixed descriptions. The writers hoped simply to hire the best actor available for each part. Casting was color-blind, with no role's race decided in advance, and the result was a racially diverse ensemble. Female roles in particular were crafted to be multi-faceted and complex.

The 2002 film Moonlight Mile supplied the reference point for the lead. Rhimes kept asking for a girl like the one in that film, and the production eventually told her they thought they could get her. Casting began with Meredith Grey, which Rhimes called a challenging role to fill.

Sandra Oh was first invited to audition for Miranda Bailey but pressed to read for Cristina Yang instead. Many actors read for Derek Shepherd. Rob Lowe was offered the part but declined, because he had already agreed to star in another medical drama, Dr. Vegas. When Patrick Dempsey read for it, Rhimes said, "he was just perfect."

Bailey was the one role written with a physical picture in mind: a tiny blonde with curly hair. Then Chandra Wilson began to speak. Rhimes said that Wilson was exactly who Miranda is, and the description was dropped.

Katherine Heigl wanted to play Izzie Stevens as a brunette but was asked to keep her natural blonde. Isaiah Washington first read for Shepherd and was cast as Preston Burke instead. Paul Adelstein, the intended Burke, had to withdraw because of commitments to the movie Be Cool. T. R. Knight signed on as George O'Malley expecting the role might be short-lived, though he liked that the character was multi-faceted. Justin Chambers completed the first-season cast as Alex Karev, who was missing from the original pilot and was added through digital editing and extra scenes.

Eric Dane had auditioned unsuccessfully for a role in the pilot. He joined in the second season as Mark Sloan, and he and Sara Ramirez, who played Callie Torres, were both given star billing at the start of the third.

  • A voice-over from Meredith Grey, or sometimes another regular, opens most episodes and often hints at the theme to come. A closing voice-over usually answers it, either by contrast or by follow-up. Between the two, a surgery typically sits near the middle of the hour.

Morning arrival at the hospital sets the tension. Residents may compete over who is assigned a challenging case, and a superior hands out the work, which sharpens the rivalry. Once a case is assigned, the doctor works toward a diagnosis, usually with an attending physician's help, and that usually ends in an operation.

Many cases quietly mirror the doctors' own struggles. Character development and relationships often take precedence over medical ethics. In the second season, Izzie Stevens bonds for weeks with a heart patient named Denny Duquette, who proposes to her. Facing his death, she cuts his left ventricular assist device to raise his place on the transplant list kept by the United Network for Organ Sharing.

The indie rock soundtrack carries the feeling. A "songtage" is a track that runs over a montage of scenes, tying storylines together with poignant lyrics, and it became a hallmark of the series.

The structure also follows medical training. Most seasons mirror the physicians' academic year, and each completed year lifts the residents to a higher level. A season finale often ends on a dramatic event such as a death or a departure.

Duquette died in the second-season finale, and Stevens landed on disciplinary probation. Yet he returned. During the fifth season she saw full hallucinations of him, a sign that she was ill, and when she became lucid he left for the last time.

  • Rhimes grew up in Chicago and first considered setting the show there. She chose Seattle instead, to distinguish it from ER, which was based in Chicago.

Fisher Plaza supplies some exteriors. The former headquarters of Fisher Communications, it houses the ABC-affiliated KOMO stations. Air ambulances in the show land on the KOMO-TV news helicopter's helipad, which suggests a hospital near the Space Needle, the Seattle Monorail and other landmarks.

Most exterior and some interior hospital shots, however, come from the VA Sepulveda Ambulatory Care Center in North Hills, Los Angeles. Shots from an interior walkway sometimes show dry California mountains behind. Most scenes are filmed at The Prospect Studios in Los Feliz, where the show occupies six sound stages. Some outdoor scenes are shot at Warren G. Magnuson Park in Seattle.

Meredith Grey's house, known as the Intern House, is real. In the story its address is 613 Harper Lane, which does not exist. The actual house stands at 303 W. Comstock St. in Queen Anne, Seattle.

Many props are working medical equipment, including a functioning MRI machine. Sarah Drew, who plays April Kepner, described the operating room scenes. The crew uses bovine organs and an actual soldering tool, and the smell is repulsive, like burning flesh. Silicone, red jello mixed with blood, and chicken fat complete the mix. She called it pretty gross.

Costumes signal rank. Attending surgeons wear navy blue scrubs and residents wear light blue. The series uses a single-camera setup and the walk-and-talk technique, which earlier series such as St. Elsewhere, ER and The West Wing popularized.

Accuracy is overseen by consultants Karen Lisa Pike, M.D., and Linda Klein, R.N. Ellen Pompeo has said the cast frequently asks the medical team about procedures and surgeries, and many cast members watched real operations to prepare. Off the set, the production staff fields a softball team that plays teams from other television shows, such as CSI: Crime Scene Investigation.

  • The third-season finale ended with Preston Burke leaving Cristina Yang at the altar on their wedding day. The same finale sent Addison Montgomery to California, where Kate Walsh's character became the lead of the spin-off Private Practice, which ran from 2007 to 2013. Burke's actor, Isaiah Washington, was also gone. ABC ended his contract after he allegedly insulted Knight with a homophobic slur during an on-set altercation with Dempsey in October 2006.

    T. R. Knight left at the end of the fifth season, citing unhappiness with his character's development and screen-time. Heigl did not return to the set after her maternity leave, and it was later confirmed she would not come back. George O'Malley then died in the sixth-season premiere, from injuries sustained saving a woman from being hit by a bus.

The sixth season ended in violence. After the merger with Mercy West Hospital, a grieving husband went on a shooting spree that injured Karev, Shepherd and Hunt. Two new residents from Mercy West, Reed Adamson and Charles Percy, were murdered.

The eighth-season finale put several doctors on a plane that crashed, killing Lexie Grey and endangering Meredith, Shepherd, Yang, Sloan and Robbins. Chyler Leigh left at her own request, with Rhimes's agreement. Mark Sloan died of his injuries in the ninth-season premiere, and Arizona Robbins lost a limb.

The survivors sued the hospital, which had put them on that plane, and they won. The payout bankrupted the hospital. With help from the Harper Avery Foundation, the survivors bought it, became its Board of Directors and were once called the "Grey-Sloan 7." They renamed it Grey Sloan Memorial Hospital in memory of Lexie Grey and Mark Sloan, replacing the Seattle Grace Mercy West name it had carried since the merger.

Sandra Oh announced in August 2013 that the tenth season would be her last. In the eleventh season, Derek Shepherd stopped to help at a car accident and was hit by a truck, then died at another hospital after his injuries were mishandled. Dempsey left with a year still on his contract, saying the show was too time-consuming and that he wanted more time with his family. On the 19th of May 2016, the night of the twelfth-season finale, Ramirez announced they would leave after their contract was not renewed.

New arrivals and spin-offs kept the hospital filled. Amelia Shepherd joined the main cast in the eleventh season from Private Practice. Ben Warren left to become a firefighter and a lead on the second spin-off, Station 19, which ran from 2018 to 2024. Andrew DeLuca died in the seventeenth season after being stabbed while pursuing a sex trafficker. In the nineteenth season, Pompeo appeared in only eight episodes, but she kept narrating and remained an executive producer.

  • The first season aired on Sundays at 10:00pm Eastern, right after Desperate Housewives. It had originally been slated for a January 2005 debut in a Monday slot. The nine episodes ran from the 27th of March to the 22nd of May 2005, as a mid-season replacement for Boston Legal.

The plan was a four-week stay in Boston Legal's time slot. High ratings changed it, and ABC kept the show there for the rest of the season. ABC Entertainment President Steve McPherson explained that without lead time or marketing money to move either show so late in the season, they would let the new drama build on its "tremendous momentum" through May.

The second season ran 27 episodes, from the 25th of September 2005 to the 15th of May 2006. It also produced two recap specials narrated by Bailey, titled "Straight to the Heart" and "Under Pressure." The third season moved to the coveted Thursday 9:00pm slot, and the show has stayed on Thursdays since. The fourth season shrank to 17 episodes because the 2007-08 Writers Guild of America strike halted production from February to April.

Ratings slipped over the years. After four seasons outside the Top 25, the show ranked No. 15 in the 2013-2014 season, its tenth, and re-entered the top five among viewers aged 18-49. The eleventh season moved to Thursday at 8:00pm.

By 2013, success had lifted Pompeo, Oh and Dempsey among the five highest-earning television actors. In January 2018, a contract renewal through the sixteenth season made Pompeo a producer. It also paid her $575,000 per episode, over $20 million a year, making her the highest-paid actress on a dramatic series.

Behind the camera, leadership changed hands. Rhimes ran the show alone for the first five seasons, then shared duties with Vernoff, then with Tony Phelan and Joan Rater. Meg Marinis took over as showrunner for the twentieth season, which premiered on the 14th of March 2024. Rhimes praised Marinis, who joined the series during its third season, telling The Hollywood Reporter that her storytelling is "a gift that continues to keep the show vibrant, compelling and alive."

  • An average score of 84% on Rotten Tomatoes, as of August 2023, sums up the critical record. Emily St. James of The A.V. Club traced a quality arc that was "all over the place." She described a steady build in the first season, a leap into phenomenon in the second, a gradual dip in the third, and some bumpy moments in the fourth, which the writers' strike interrupted. In her view, the climb began again in the fifth.

Early reactions were mixed. Gary Levin of USA Today called the show one of the top series on television, and Newsday said you simply cannot stop watching. Tom Shales of The Washington Post found the first season reminiscent of ER and more a matter of commercial calculation than a fresh idea. Maureen Ryan of the Chicago Tribune called it the new Friends.

Samantha Highfill of Entertainment Weekly named the second season the show's best to date. She noted that it had entered the cultural lexicon through phrases like "McDreamy," the nickname the residents gave Derek Shepherd. She also described it as by turns a good soap, a good romantic comedy and a good medical drama.

The awards record includes a Golden Globe for Best Television Series - Drama and 38 Primetime Emmy nominations, two of them for Outstanding Drama Series. Individual cast members have also been honored. In streaming, the show ranked 10th among on-demand programs. Netflix signed a deal in 2009 to stream episodes, and in Germany and the United Kingdom the full series is available on Disney+.

The hospital shows no sign of closing. The twenty-second season premiered on the 16th of October 2025, and on the 30th of March 2026 ABC renewed the series for a twenty-third.
