On the 17th of October 2021, a fan fell from an upper level of Chase Center during a Phish concert and died from his injuries. Two other fans were also injured in fall incidents at the same event and survived. Following an anonymous complaint about guardrails and staircases, the San Francisco Department of Building Inspection inspected the arena and on the 21st of October deemed it compliant with city building codes.
